These documents are fillable PDF forms, meaning, they may be filled-in using your computer. Click with your cursor above any available line on a form and you will be able to type in the requested information. You may also click on any of the checkboxes to mark them. Once all fields are completed, print out the document on your printer and physically sign the document where indicated, if necessary.
